When presenting a valid Chronic Disease Management Plan!
Chronic Disease Care Plans
Chronic Disease Management plans from your GP are eligible for Medicare rebates. Our admins will assist you with Medicare rebate of $60.35.
At the end of your session, you will receive an invoice through email that allows you to claim rebates through My Gov App or online.
Private Clients
Advance payment is required for private clients, which will be refunded when allowing 24hrs before appointment cancellation.
At the end of your session, you will receive an invoice through email that allows you to claim rebates through your private health insurer.
